I'm not sure this is fiction. The first 17% of the story is extended pontification by the author on French, English, and Italian passion. If you slog through it, you may not care about the story.
The story itself concerns Pope Paul IV, who allowed his nephews to run roughshod over Rome at the time when the Church ruled Rome (and therefore Italy.) The corruption and betrayals are familiar to anyone who has read any Italian history. The Duchess is an innocent who gets ground up in the process.
